
As Program Manager of the Health Coalitions, Jeffrey Freeman supports the African American and Latino Health coalitions and the New American Advisory Group to improve the health status of residents in the nine-county Finger Lakes region. During his time at Common Ground Health, he has also analzyed trends within the High Blood Pressure Registry; researching health disparities by race/ethnicity, social economic status, age and gender; and helping reduce the number of uninsured citizens.
Prior to joining Common Ground Health, Freeman had more than a decade of experience working with Excellus Blue Cross Blue Shield and Blue Cross Blue Shield of Western New York, where he was involved with preparation of the annual reports for the state Department of Insurance and HEDIS; analysis for the on-going Emergency Room/Urgent Care Transition Project; and quarterly reporting of electronic prescription utilization.
In addition to his work with the Common Ground Health, Jeff volunteers on several projects geared towards youth leadership and education in the Rochester community.
A Class of 2011 graduate of the Greater Rochester United Way’s African American Leadership Development Program, Freeman reinvests his time each year to the success of the program.
