March 2019

Working together to improve health

Common Ground Health’s year in review provides a snapshot of our collaborative approach to health improvement. You’ll read about new initiatives to support the social and emotional health of children, campaigns to make streets safer for walkers and cyclists, and efforts to create a culture of health in congregations, barbershops, salons and schools.

You’ll also learn about programs to help medical practices adopt a team approach to care and use clinical data to drive continuous improvement.

This work is made possible through our 245 partnering organizations and the hundreds of individuals who serve on collaborative tables and support initiatives.

Partner spotlight:
UR Medicine and Corning Health

Kudos to the Developmental and Behavioral Pediatrics team at Golisano Children’s Hospital for creating a Southern-Tier specific website featuring resources and services for children with special needs. Funded by Corning Health, the website focuses on supports in Chemung, Schuyler and Steuben counties. These include resources on advocacy, behavioral health, food and housing, health, parent support, recreation, transportation and vocation/employment.

“Parents of children with special needs, particularly those living in rural areas, need help connecting with specialists,” said Wade Norwood, CEO of Common Ground Health. “I encourage people to share this resource.”
