Why is a farm-rich region so hungry?

 Food Farms and Health apples

The Finger Lakes region has abundant fruit and vegetable farms. Yet 22 percent of the population goes without eating healthy, fresh food, which harms health outcomes.

The Food, Farms and Health Symposium April 11 explored the roots of this problem and sparked a dialogue about barriers to fresh food access.

“Economically, environmentally, physically and emotionally, our individual and community health is linked,” said Anne Ruflin, chief planning officer for Common Ground Health, which cosponsored the symposium, along with Foodlink and S2AY Rural Health Network.

Succeed in the new value-based world

From MACRA and MIPS to SIM and SHIP, federal and state programs aimed at paying for health outcomes instead of procedures can be dizzying.

That’s where our expert consultants can help. Common Ground Health can provide no cost assistance to practices looking to participate in the state’s Advanced Primary Care model or prepare for new quality reporting requirements.

“We help practices create a culture of continuous improvement, a shift that is foundational to meeting the new demands of outcome-based payment,” said William Brien, director of Practice Transformation for Common Ground Health.

Event explored faith, mental health

Dr. Linda Clark opens the Faith and Medicine Conference.

The stigma surrounding mental illness and substance abuse prevents many people from receiving treatment, said leaders at the Faith and Medicine Conference April 29.

“If this were high blood pressure, we’d deal with it,” Dr. Cheryl Kodjo, said during a panel discussion. “How is this any different?”

The event, a collaboration of several local organizations, highlighted issues surrounding substance abuse, post-traumatic stress disorder, depression and other mental illnesses.

Partner spotlight: S2AY

Kudos to our population health innovators for bringing a new state grant to our region.

In the Finger Lakes, S2AY Rural Health Network is leading a $2.5 million initiative to help providers connect patients with services outside of clinical settings.

“The region is fortunate to have such outstanding leadership on population health,” said Trilby de Jung, CEO of Common Ground Health.

Event: Planning can cut readmissions

Reducing readmissions using discharge planning and technology and attracting and retaining a skilled workforce will be discussed at the Cracking the Code on Healthcare Spotlight Event June 2 at Locust Hill Country Club. Learn more here.
